agent-army: Fable/Opus commander + power levels for swarm model tiers

- Commander is the session model (Fable or Opus), never delegated
- New Power Level step: Max Power / Heavy / Balanced / Economy set the
  Opus/Sonnet/Haiku mix for Layer 1 and Layer 2, asked alongside army size
- model: passed explicitly on every Agent call; per-file escalation allowed
- Audit waves always run on Opus; notify waves can drop to Haiku
- New mandatory rule + gate check + anti-pattern against silent model drift
