Make Claude 4.x routing version-aware; add Opus 4.8 (v1.7.0)

Opus 4.8 is now the default Claude model, but the skill was pinned to a
4.7-only worldview and asserted stale, version-specific facts as hard rules.

- Restructure Claude LLM routing into version-aware blocks: durable advice
  generalized across 4.6/4.7/4.8, new Opus 4.8 (current default) profile,
  Opus 4.7 kept and clearly labeled.
- De-hardcode the Claude Code "xhigh effort" note. Effort/thinking depth is
  harness-managed on current Opus models; the prior claim was 4.7-specific
  and unverifiable for 4.8, so it's generalized rather than replaced with a
  fabricated 4.8 number.
- Template M renamed "Opus 4.7 / 4.8 Task Brief" (heading, TOC, body).
- Pattern 36 covers 4.7 and 4.8.
- Fix stray "schema" |" fragment dangling off the pattern 37 row in
  patterns.md.
- Bump version 1.6.0 -> 1.7.0 and add README history entry.
