Add self-contained visual companion server for Draft phase

Port the browser-based visual companion from superpowers into Blueprint
with Blueprint branding and paths (.blueprint/companion/ instead of
.superpowers/brainstorm/). Zero external dependencies — Node.js only.

Components:
- server.cjs: HTTP + WebSocket server with file watching and live reload
- start-server.sh: Launch script with platform detection (macOS, Windows, Codex)
- stop-server.sh: Graceful shutdown with signal escalation
- frame-template.html: OS-aware dark/light theme with CSS helpers
- helper.js: Client-side WebSocket + selection tracking (XSS-safe, no innerHTML)

Update references/visual-companion.md to point to actual scripts.
