docs: sync skill catalog in README and llms.txt
Both files had drifted out of sync with skills/ over several releases:
missing 8 (README) / 15 (llms.txt) skills, and 4 entries still using
pre-v7.0.0 names (dt-obs-{android,flutter,ios,react-native}).
