Fix groom workflow: bypass MCP integrity filter with gh api (#627)

* Fix groom workflow: use gh api to bypass MCP integrity filter

The MCP gateway's integrity policy intermittently blocks access to issue
comments (report_incomplete: 'Resource issue:dotnet/skills#288 has lower
integrity than agent requires'). This caused issue #625.

Fix: Add 'gh' to allowed bash tools and instruct the agent to use
'gh api --paginate' for fetching comments. This bypasses the MCP gateway
entirely and also provides node_id fields required by hide-comment
(which the MCP issue_read tool did not return).

* Address PR review: fix --paginate JSON, add author filter, narrow guideline scope
