docs(maggy): add self-healing system design to RFC (Section 15)

detection: ServerHealthCollector (process/endpoint monitoring),
FrustrationCollector (chat analysis with target taxonomy: app_bug vs
output_quality vs task_difficulty), and UIAnomalyCollector. Includes
weighted scoring dimensions, threshold actions, and triage engine design.
