Alex Newman e2d309b372 fix(hooks): anchor project names to Claude project dir (#4055) ...

Prefer CLAUDE_PROJECT_DIR when normalizing Claude Code hook payloads so SDK/subagent temp cwds are not stored as the project identity. getProjectName stays authoritative for explicit worker/transcript cwds.

Rebase of #3439 onto main.
