v3.16.0: provider-switch model refresh + remove GLM Vision

Refresh provider-switch model pins to current flagships (GLM 5.2, Qwen 3.7-plus, MiniMax-M3; DeepSeek unchanged; OpenRouter catalog refreshed) and remove the unused GLM Vision feature entirely (design-to-code skill, trial, two agents, and the Z.ai image-gen provider). Also fixes a mobile docs table that overflowed the viewport.
