Claude Code reads .claude-plugin/plugin.json and has never read claude-code.json (the string does not appear in the 2.1.241 binary). Without the manifest the plugin name falls back to the install directory, which in a marketplace cache is the version — reproduced with the real plugin under a directory named 27.0.0-beta.99: skills and agents registered as `27.0.0-beta.99:axiom-swiftui` and both /axiom:axiom-performance and /axiom:health-check returned "Unknown command". Current Claude Code masks this because the marketplace entry supplies the name, so correct namespacing rested on that rescue plus the accident that the repo plugin directory is called `axiom`. Declares no `hooks` key, leaving hooks/hooks.json convention discovery intact (verified: the SessionStart injection still fires). Closes GH #53.
