Boden Fuller 1b0ba1cdfb chore(docs): reconcile doc/ops debt waves A–F

Align br-first tracker truth, RPI terminology, stale architecture surfaces,
new drift gates, gate deferral drain (13→11), and audit artifacts so agents
and operators share one hookless map with mechanical acceptance.
