Ben Follington 49e9416f79 Restructure terma skills to folder/SKILL.md format and bump versions ...

Plain .md files in skills/ weren't being detected by the plugin loader.
Moved all 6 core skills (code-review, ideate, implement, orient, plan,
research) into the required folder/SKILL.md structure with frontmatter
and updated @-reference paths.

Versions: terma 1.1.0, tsal 1.1.0, notes 1.0.4
